Minuteful Kidney service for patients with diabetes (and/or other conditions)

The NHS has commissioned Healthy.io, as part of a national NHS programme, to deliver a service providing at-home urine test kits so patients at risk of chronic kidney disease can conduct this important test at home using a smartphone.  If eligible you will be contacted by the practice to establish if you wish to engage with the programme.  Please see more information under the privacy notice section.
