Patients 16 or over can now access a whole host of services through our on-line Patient access app which works on your smartphone, PC, tablet or any device that can connect to the internet. It works on both Windows, Apple and Adroid devices.
Once installed, you will be able to request two levels of access which are:
- Make an appointment with a GP or Practice Nurse, cancel or even reschedule an appointment and check the time of an appointment you have already booked.
- Order a repeat prescription – any time day or night! We can even arrange for the script to be delivered to your chosen chemist for collection, saving you even more time!
- Check the Allergies we have recorded on your notes.
Level 2: Everything within Level 1 and also limited access to:
- Recent Immunisations
Which is Right for Me?
Level 1 Access Only
If you only feel you need to have access to level 1, your application will be able to be processed quickly by completing the application form.
Level 2 Access
Applications from those patients who would like access to more detailed information, requesting level 2 access, may take much longer. The practice also reserves the right to redact information that may lead to the harm of yourself or others or to refuse level 2 access althogether, although the Practice will explain why.
Before you apply for level 2 the are some other things to consider:
- Forgotten history
There may be something you have forgotten about in your record that you might find upsetting.
- Abnormal results or bad news
You may see something that you find upsetting to you. This may occur before you have spoken to your doctor or while the surgery is closed and you cannot contact them.
- Choosing to share your information with someone
It’s up to you whether or not you share your information with others – perhaps family members or carers. It’s your choice, but also your responsibility to keep the information safe and secure.
- Coercion If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.
Applying for Level 1 or 2 Access.

You will need to provide two forms of documentation to prove your identity, one of which must contain a photograph (such as a passport or driving licence) and one confirming your address, such as a bank statement. Utility bills may be accepted if none of the above are available, subject to discression. Once your registration has been processed, you will need to present the information to gain the document that allows access to the system.

Please note that the service is only available for patients aged 16 or over and each patient in a family must register separately.
Patients already registered for level 1 will need to re-register for level 2 access.
Not all appointments are available on-line: same day triage, urgent appointments, treatment room, minor surgery, midwife, district nursing and some disease management clinics will still only be available by contacting the surgery.

Once I am Registered
Once you have been registered you will need to use the form given to you by the Practice to register on the site. The web address is printed on your sheet.
